Job summary

Senior Community Care of Michigan - PACE, a Volunteers of America National Services program, seeks a full-time Nurse Practitioner to deliver primary care to frail elderly participants. You will assess, plan, and coordinate care with the Interdisciplinary Team.

Under Medical Director supervision, you will perform assessments, develop care plans, and teach patients and caregivers. This is a fully onsite role with occasional weekend coverage and local travel to participants' homes.

Qualifications

• Licensed RN in the employment state.
• Nationally certified as a Nurse Practitioner.
• Experience with frail and elderly populations preferred.

Responsibilities

• Provide primary care to Senior Community Care program participants under Medical Director supervision.
• Perform physical assessments and semi-annual reassessments; implement care plans.
• Coordinate with Interdisciplinary Team to deliver comprehensive, patient-centered care.
