Job summary

Curative's PACE program in the Greater Denver Region is seeking a Geriatrics Advanced Practice Provider (NP/PA) to join our interdisciplinary team and deliver person-centered care to seniors.

You will experience a low-volume schedule with 5 face-to-face encounters per day and a dedicated remote administrative day each week. Enjoy true team-based medicine with direct leadership access and a competitive base salary.

Qualifications

• 2+ years of experience in geriatrics, internal medicine, family practice, SNF or home care preferred.

Responsibilities

• Mon–Wed (On-Site): clinic visits, acute assessments, and daily IDT meetings.
• Thu (Community): local nursing facility/SNF/home visits.
• Fri (Remote): remote charting, care coordination, and follow-ups.
• Low volume with 5 face-to-face encounters per day; nurse triage handles first-line calls; provider escalation is rare.

Our Program of All-Inclusive Care for the Elderly (PACE) center in the Greater Denver Region is looking for a compassionate, skilled Advanced Practice Provider (NP or PA) to join our collaborative Interdisciplinary Team (IDT).

We are actively hiring for an immediate onboarding window to ensure a comprehensive, intentional overlap period with departing team members—giving you a smooth handoff, full panel continuity, and a stress-free orientation.

Requirements

• AGPCNP, FNP, ACNP, or NCCPA certification.


• 2+ years experience in geriatrics, internal medicine, family practice, SNF, or home care preferred (standout new grads with strong RN/rotation background considered).


• Competitive Base Salary (based on experience) + Call Stipends.
