Job summary

AltaMed Health Services Corporation in Huntington Park, CA is seeking a Staff Physician (Internal Medicine, Geriatrics) to deliver comprehensive primary care within the PACE program. You will work across the IDT and coordinate care for a defined panel of participants.

You will provide patient-centered services in the PACE center, participants’ homes, via telehealth, and SNFs, ensuring quality and regulatory compliance.

Qualifications

• Board-certified or eligible in Internal Medicine, Family Medicine, Emergency Medicine or Geriatrics.
• Active CA medical license required; DEA license required.
• Current BLS certification or higher.

Responsibilities

• Deliver comprehensive primary care for PACE participants within an IDT.
• Coordinate care across settings: PACE center, homes, telehealth and SNFs.
• Promote wellness, prevent illness, and manage a defined panel of participants.
• Ensure regulatory compliance and collaborate with specialists.
