Oak Street Health is seeking a Primary Care Physician to provide equitable, value-based healthcare to local Medicare populations at our neighborhood clinics. You’ll work in a center supported by a large care team, focusing on quality and preventive care rather than volume.

Qualifications

• MD/DO degree from an accredited medical school.
• Board certification in Internal Medicine or Family Medicine (board-eligible considered).
• Active, unrestricted State License.
• Active DEA license.
• US work authorization.

Responsibilities

• Assess and diagnose patients at local clinics.
• Oversee, direct, and administer primary care.
• Prescribe and manage medications.
• Maintain Patient EMR data via canopy and greenway.
• Collaborate with regional and central leadership to meet quality goals.
• Work with Practice Managers to direct and manage the center care team.
• Other duties as assigned.
