Oak Street Health in Charleston, SC seeks a Primary Care Physician to deliver equitable value-based care for local Medicare populations within our neighborhood centers. Our physicians work in a single medical center supported by large care teams to focus on quality outcomes and patient experience.

New graduates are encouraged to apply, and highly qualified candidates may be considered for a Center Medical Director position.

Formación

• MD or DO degree.
• Board-certified in Internal Medicine or Family Medicine (or eligible).
• US clinical practice license required.

Responsabilidades

• Assess and diagnose patients at local clinics.
• Oversee, direct, and administer primary care.
• Prescribe and administer pharmaceutical treatments and medication.
• Maintain Patient Electronic Medical Record data via canopy and greenway.
• Collaborate with leadership to meet health quality goals.
• Work with Practice Managers to direct and manage the center care team.
• Other duties as assigned.

Conocimientos

Active medical license

Board certification in Internal or Fam

US work authorization

Geriatrics experience
