Allaire Health Services, LLC is seeking an Attending Physician to deliver comprehensive medical care for residents in long-term care, subacute, ventilator, and behavioral health units in New Jersey. You will round regularly, lead multidisciplinary teams, and implement evidence-based interventions to improve outcomes and reduce avoidable hospitalizations.

Formación

• MD or DO from an accredited medical school.
• Board Certified/Eligible in Internal Medicine, Family Medicine, or Geriatrics.
• Active, unrestricted state medical license and current DEA registration.
• Experience in post-acute/long-term care, ventilator care, or behavioral health preferred.
• Proficiency with EMR systems and clinical documentation.
• Demonstrated ability to collaborate with NPs/APPs, nursing, rehabilitation, and admin staff.
• Current BLS/ACLS (or willingness to obtain).

Responsabilidades

• Provide direct, comprehensive medical care to residents in long-term, subacute/post-acute, ventilator, and behavioral health settings.
• Round regularly and conduct evaluations with timely follow-ups for continuity of care.
• Lead multidisciplinary care team meetings and contribute to care plans.
• Manage chronic and complex conditions with decisive interventions and appropriate therapies.
• Communicate clearly with nursing staff, NPs/APPs, patients, families and consultants.
• Coordinate care with facility nursing, rehabilitation teams, leadership, and NP.
• Discuss goals of care and palliative options with patients and families.
• Promote healthy aging through preventive care and proactive treatment plans.
• Document accurately in the EMR and maintain strong documentation habits.
• Develop strategies to reduce avoidable transfers and readmissions.
