Job summary

Allaire Health Services, LLC seeks an Attending Physician to deliver comprehensive medical care for residents across long-term care, subacute/post-acute, ventilator, and behavioral health units. You will round regularly and lead multidisciplinary teams to implement evidence-based interventions, reducing avoidable hospitalizations.

Ideal candidates hold an MD/DO with Board Certification/Eligibility in Internal Medicine, Family Medicine, or Geriatrics, with an active license and DEA.

Qualifications

• MD or DO from an accredited medical school; board certified/eligible in Internal Medicine, Family Medicine, or Geriatrics.
• Active, unrestricted state medical license and current DEA registration.
• Experience in post-acute/long-term care, ventilator care, or behavioral health preferred.
• Proficiency with EMR systems and clinical documentation.
• Demonstrated ability to collaborate with NPs/APPs, nursing, rehabilitation, and administrative staff.
• Current BLS/ACLS or willingness to obtain as required by facility.

Responsibilities

• Provide direct, comprehensive medical care to residents in long-term, subacute/post-acute, ventilator, and behavioral health settings.
• Conduct routine rounding, evaluations, and timely follow-ups to ensure continuity of care and early intervention.
• Lead and participate in multidisciplinary care team meetings; contribute to individualized care plans.
• Aggressively manage chronic and complex conditions; respond to status changes with appropriate therapies.
• Communicate clearly with nursing staff, NPs/APPs, patients, families, and consulting providers.
• Collaborate with facility nursing, rehabilitation teams, leadership, and assigned NP to coordinate care.
• Discuss goals of care and palliative options; support shared decision-making.
• Promote healthy aging through preventive care and proactive treatment plans.
• Document accurately in the EMR; maintain strong clinical documentation habits.
• Help develop and implement proactive strategies to reduce avoidable transfers and readmissions.
