Allaire Health Services, LLC seeks an Attending Physician to deliver comprehensive medical care for residents across long-term care, subacute, ventilator, and behavioral health units. You will round regularly, lead multidisciplinary teams, and implement evidence-based interventions to improve outcomes and reduce hospitalizations.

Competenze

• MD/DO from an accredited medical school; board certified/eligible in Internal Medicine, Family Medicine, or Geriatrics.
• Active, unrestricted state medical license (or eligibility to obtain) and current DEA registration.
• Experience in post-acute/long-term care, ventilator care or behavioral health preferred.
• Proficiency with EMR systems and clinical documentation.
• Ability to collaborate with NPs/APPs, nursing, rehab and administrative staff.

• Provide direct, comprehensive medical care to residents in geriatrics and long-term care settings.
• Conduct rounds, evaluations, and timely follow-ups to ensure continuity of care.
• Lead multidisciplinary care team meetings and contribute to individualized care plans.
• Manage chronic and complex conditions with decisive interventions.
• Communicate clearly with nursing staff, patients, families, and consultants.
• Coordinate with nursing, rehabilitation, leadership, and NP to optimize care.
• Discuss goals of care and palliative options with patients and families.
• Promote preventive care and proactive treatment plans to aging residents.
• Document accurately in the EMR and maintain strong clinical documentation.
• Develop strategies to reduce avoidable hospital transfers and readmissions.
