Job summary

Newave Care is seeking an Attending Nurse Practitioner to provide comprehensive medical management across long-term care, ventilator, behavioral health, and subacute/post-acute settings. You will round regularly, coordinate care with interdisciplinary teams, and implement evidence-based interventions to reduce avoidable hospitalizations.

Qualifications

• Board certification as an AGNP, GNP, or FNP is required.
• Current, unrestricted NP license in the practicing state; national cert.
• Current BLS and ACLS certifications.
• Experience in long-term care, subacute rehab, ventilator care, or chronic disease management preferred.
• Strong clinical assessment, diagnostic skills, and clear documentation.
• Familiarity with EMR systems and post-acute protocols.
• Ability to collaborate with physicians, nurses, rehab and admin staff.
• Excellent communication and organizational abilities.

Responsibilities

• Provide comprehensive medical management for residents in long-term care and related units.
• Conduct regular rounding to ensure continuity and timely follow-ups.
• Manage chronic conditions and respond to status changes with appropriate care.
• Prevent avoidable hospitalizations through early intervention.
• Collaborate with nursing, rehab, leadership, and physicians to optimize care plans.
• Discuss goals of care and palliative options with patients and families.
• Promote preventive care and wellness in the elderly population.
• Document thoroughly in the EMR with high standards.
• Participate actively in interdisciplinary care meetings and planning.
