Job summary

Curative's Pace center in the Greater Denver Region is seeking an Advanced Practice Provider (NP/PA) to join a collaborative IDT focused on geriatric care. Enjoy a low-volume, high-impact schedule with 75-80 seniors, regular IDT meetings, and a weekly remote admin day.

Qualified candidates have Colorado NP licensure, DEA, and AGPCNP/FNP/ACNP or NCCPA credentials with 2+ years in geriatrics or related fields. Comprehensive benefits, sign-on, and relocation support available.

Qualifications

• Active Colorado NP license and DEA certification required.
• Advanced practice certifications (AGPCNP/FNP/ACNP or NCCPA) required.
• 2+ years of geriatrics, internal medicine, family practice, SNF, or home care experience preferred.

Responsibilities

• Manage a panel of 75-80 seniors with a gradual ramp-up.
• Collaborate with an interdisciplinary team to coordinate care.
• Participate in team meetings and care planning activities.
• Provide care with a focus on relationship-based, long-term outcomes.
