Job summary

Habitat Health in Chicago seeks a Medical Director to lead clinical quality, safety, and participant experience at the PACE center. You will balance direct patient care with team development, program leadership, and fiscal stewardship in alignment with our mission.

The role requires an MD/DO, active licensure, 3+ years in geriatrics or related care, and proven ability to lead interdisciplinary teams within value-based care models. Travel locally is required.

Qualifications

• MD or DO with active, unrestricted state licensure; DEA license; board certification in Internal Medicine, Family Medicine, Geriatrics, or related specialty.
• 3+ years of clinical experience caring for older adults in PACE, geriatrics, SNF, hospice, home-based care, managed care or similar setting.
• Experience leading clinicians and interdisciplinary teams through coaching, mentorship, and teaching; experience with value-based or capitated care models.

Responsibilities

• Direct Clinical Care & Panel Management: provide care as primary clinician for a panel and supervise other clinicians; ensure documentation, coding integrity, and medication safety.
• Clinical Program Leadership: oversee clinical quality, safety, outcomes, and participant experience; drive improvements using data; ensure CMS PACE compliance.
• Teaching, Coaching & Staff Development: educate IDT members, provide mentorship and timely feedback; foster staff engagement and inclusive culture.
• Organizational Leadership & External Partnerships: serve as dyad partner to Executive Director; liaise with hospitals and community providers; influence center performance.
