Responsibilities

• Provides diagnosis, treatment, and ongoing management for PACE participants and hospice patients.
• Delivers primary care in clinic and, when needed, in the participant's home.
• Participates in the clinical call rotation; phone-only after-hours coverage.
• Contributes to interdisciplinary team conferences and care planning.
