Job summary

CareMed seeks a Wound NP to provide comprehensive wound care services for residents in skilled nursing facilities. The role includes prescribing medications, ordering and reviewing treatments, and coordinating with the care team to optimize patient outcomes.

The ideal candidate holds an active Kentucky NP license, board certification, and experience with wound care, including chronic and infected wounds, with EMR proficiency and a collaborative approach to interdisciplinary care.

Qualifications

• Active KY NP license and board certification.
• DEA licensure is optional but advantageous.
• Extensive wound care background across chronic/acute wounds.

Responsibilities

• Provide wound care services for long-term care patients in skilled nursing facilities.
• Prescribe medications, including controlled substances per KY law.
• Order, review, and follow up on treatments and diagnostics.
• Communicate with nursing home staff regarding patient condition changes.
• Coordinate with the primary care physician and care plan.
