Job summary

CareMed Health Solutions is seeking a Wound Care Nurse Practitioner to deliver specialized wound services for residents in long-term care facilities. The role involves prescribing medications, ordering tests, and coordinating with nursing staff and the primary care physician to ensure comprehensive care.

The ideal candidate holds a Kentucky NP license with board certification and experience in wound care, preferably within long-term care settings.

Qualifications

• Current, unrestricted NP license in Kentucky.
• Board Certification.
• Wound care experience in long-term care.
• Familiar with wound care products and DME; collagens, alginates, foams, silvers.

Responsibilities

• Provide wound care services for long-term care patients in skilled nursing facilities.
• Prescribe medications, including controlled substances in accordance with KY Law.
• Order, review, and follow-up with treatments and diagnostic testing.
• Communicate with nursing home staff on changes in condition and care plan.
• Collaborate with the primary care physician and coordinate patient care as part of an interdisciplinary team.
