Zusammenfassung

Total Life in Dallas–Fort Worth is seeking a Longevity Ambassador to represent our mission in the community. You will conduct in-person wellness assessments for older adults, build relationships with physician practices and residential partner sites, and be the friendly face who introduces Total Life’s emotional wellness approach.

This role involves hosting longevity screening events, traveling to partner sites, coordinating with the virtual therapy team, and logging outreach in our CRM.

Qualifikationen

• Extroverted, cheerful and energized by community work.
• Comfortable entering physician offices and residential sites without an appointment.
• Warm, patient, and genuinely interested in older adults.
• Healthcare background or currently enrolled healthcare student (MA, CNA, nursing, social work, public health, gerontology).
• Available a minimum of 10 hours per week.
• Reliable transportation for local travel.
• Comfortable using a CRM and standard phone and email tools.
• Able to transport and set up booth materials.
• Must pass a criminal background check.

Aufgaben

• Conduct in-person wellness assessments for older adults to evaluate emotional, cognitive, and social well-being.
• Represent Total Life in the community and residential partner sites as the local point of contact.
• Build and maintain referral relationships with physician practices, senior living communities, and home health agencies across territory.
• Host Total Life's longevity screening events at partner sites and help attendees book first appointments.
• Collaborate with the virtual therapy team to ensure smooth transitions into ongoing care.
• Maintain accurate documentation and log outreach activity in CRM on the day of activity.
• Participate in team meetings and contribute to improving community-based programs.
