Job summary

Total Life seeks a Longevity Ambassador to represent us in the Greater Chicago area, conducting in-person wellness assessments for older adults and building relationships with physician practices and residential partner sites. You’ll be the welcoming face in the community, hosting longevity screenings and coordinating with our virtual therapy team to ensure smooth transitions into ongoing care.

Ideal candidates are extroverted, enthusiastic about elder care, and able to work a minimum of 10

Qualifications

• Genuinely extroverted and cheerful; comfortable starting conversations with people.
• Willingness to visit physician offices and residential communities without appointments.
• Warm, patient, and genuinely interested in older adults.
• Healthcare background (MA, CNA, nursing, social work, public health, gerontology) or current enrollment.
• Available a minimum of 10 hours per week.
• Reliable transportation for local travel.
• Able to use CRM and standard phone/email tools.
• Able to transport and set up booth materials.
• Must pass a criminal background check.

Responsibilities

• Conduct in-person wellness assessments for older adults to evaluate emotional, cognitive, and social well-being.
• Represent Total Life in the community and partner sites as the local contact for clients and partners.
• Build and maintain referral relationships with physician practices and senior living communities.
• Host longevity screening events at partner sites and help attendees book their first appointment.
• Collaborate with the virtual therapy team to ensure smooth transitions into care.
• Log outreach activity in the CRM the same day.
• Participate in team meetings and contribute to program improvements.
