Job summary

Central Valley PACE seeks a Speech-Language Pathologist to provide per-diem services across PACE Center, nursing facilities, assisted living, and participants' homes. Training will occur at the CV PACE Center in Modesto until the Stockton location is operational.

The role includes annual assessments, care planning with the IDT, and thorough documentation; per-diem schedule is Monday–Friday, 8am–5pm, with flexibility to meet participant needs.

Qualifications

• Master’s degree in speech-language pathology.
• Licensed by CA Board Examiners in Speech-Language Pathology and Audiology.
• Current BLS CPR certification.
• Minimum one year experience working with the frail or elderly population.
• Proficient in Microsoft Office applications; advanced Microsoft Excel experience.

Responsibilities

• Provide Speech Language Pathology services in the PACE Center, Nursing and Assisted Living Facilities and in the homes of participants.
• Perform initial and periodic assessments minimally every six months; communicate participant changes to the team and plan of care.
• Document notes in the medical record; maintain concise and relevant charts.
• Participate in IDT care planning and coordination for 24-hour care delivery.
• Contribute to quality improvement activities and data collection.
