Job summary

Golden Valley Health Centers is seeking a Registered Dietitian to provide nutrition care and education to Central Valley PACE participants. The role includes assessments, care planning, and collaboration with physicians and the interdisciplinary team, primarily at the Modesto center until Stockton opens.

This per diem position offers compensation of $48.00–$52.00 per hour and requires a CA driver's license, geriatric experience, and proficiency in nutrition guidelines and documentation.

Qualifications

• Knowledge of adult and geriatric health, nutrition, motivational interviewing and adult learning.

Responsibilities

• Performs nutritional assessments and reassessments per nutrition standards.
• Provides nutrition education to participants, caregivers, staff and community as ordered by physician.
• Collaborates with the care team to implement participant education and 24-hour care coordination.
• Documents changes in medical records and communicates them to team members.
• Participates in IDT meetings and care planning; adheres to CMS standards.
