Mission Healthcare in Modesto, CA, is seeking a Speech-Language Pathologist for per diem/PRN work in home health. You will evaluate patients, develop plans of care, and provide therapy for speech, language, voice, and swallowing disorders.

You will coordinate with physicians and the clinical team, document progress, supervise home health aides, and stay current with best practices. State license, CCC-SLP, and CPR are required; one year of home health experience is preferred.

Qualificações

• Master’s degree in Speech-Language Pathology from an accredited program.
• CCC-SLP certification from ASHA and state licensure.
• At least one year of clinical experience in speech-language pathology.
• Current BLS (CPR) certification and strong communication/documentation skills.

Responsabilidades

• Conduct thorough speech-language evaluations and assess the home environment to support therapy goals.
• Develop and implement personalized plans of care for patients with speech, language, voice, and swallowing disorders.
• Provide therapy using specialized techniques and equipment tailored to each patient’s needs.
• Coordinate care with referring physicians and other clinical team members.
• Document evaluations, treatment plans, and progress in accordance with agency policies.
• Supervise home health aides when related to the plan of care.
