Job summary

Family Health Centers of San Diego (FHCSD) is seeking a Per Diem Speech Language Pathologist to provide evaluation and treatment for patients with speech and language delays or disorders, including adults 55+ enrolled in PACE. The SLP will support early intervention, consult with the multidisciplinary team, and contribute to education for staff and patients.

Responsibilities include screening, diagnosing, and treating disorders; documentation; and coordinating care across sites in the San Diego

Qualifications

• Master's degree in Speech Pathology required.
• California State licensure as Speech Language Pathologist required; license must be maintained for duration of employment.
• C.C.C. (ASHA) in Speech Pathology preferred.
• CPR Certification is required.

Responsibilities

• Screens and diagnoses speech and language disorders for individuals 55+ enrolled in PACE.
• Provides treatment and therapy for PACE participants with various communication disorders.
• Conducts home visits and coordinates scheduling with families and caregivers.
• Develops and documents treatment goals and plans; communicates with the multidisciplinary team.
• Completes documentation accurately and on time, meeting insurance requirements.
• Manages scheduling to keep appointments filled and efficient.

Education

Master's degree in Speech Language Pathology

C.C.C. (ASHA) in Speech Pathology
