Job summary

Golden Valley Health Centers is seeking a Per Diem Speech Language Pathologist to support Central Valley PACE in Stockton, CA. You will deliver SLP services across the PACE Center, Nursing and Assisted Living facilities, and participant homes.

The role requires a Master’s in SLP, CA licensure, and at least 1 year with frail or elderly populations. You’ll participate in QA activities, maintain records, and collaborate with an interdisciplinary team to optimize care.

Qualifications

• Valid CA Driver’s License with acceptable driving record and vehicle insurance.
• Ability to work under stressful conditions and stay organized.
• Excellent written and verbal communication with accurate records.
• Integrity, discretion and objective problem solving.
• Data collection skills and ability to work with diverse teams.
• Skills in building effective relationships with participants, coworkers and medical staff.
• Ability to identify and recommend problem resolution.
• Knowledge of safety and infection control in healthcare facilities.
• Experience with QA/PI activities and staff education using adult learning concepts.
• Proficiency in MS Office; advanced Excel experience required.
• Maintain professional speech pathology affiliations and align with GVHC values.

Responsibilities

• Provide SLP services to Central Valley PACE participants in centers, facilities and homes.
• Conduct initial and periodic assessments (min every 6 months) and document changes.
• Communicate participant changes to the care team and plan of care.
• Coordinate for 24-hour care delivery and document in the health record.
• Participate in IDT care planning and quality improvement activities.
