Job summary

OPCO Skilled Management in Independence, MO, is seeking a dedicated Social Worker to support residents in a skilled nursing setting, promoting dignity and independence, and coordinating psychosocial services in line with NASW code of ethics. Responsibilities include completing social histories, guiding care plans, documenting in compliance with laws, and supporting residents’ self-determination.

Requirements: state license, a BSW or related degree, and at least one year in healthcare social

Qualifications

• Bachelor’s degree in Social Work or Human Services field required.
• Certified, licensed, or registered in the state of practice required.
• Minimum one year of supervised social work experience in a healthcare setting.

Responsibilities

• Complete social history and psychosocial assessment for each resident.
• Identify social, emotional, and psychological needs and guide care plans.
• Maintain accurate documentation compliant with regulations and policies.
• Involve residents in education about alternatives and self-determination.
• Develop interdisciplinary plans of care addressing psychosocial needs.
• Support compliance with NASW Code of Ethics and governing regulations.
