Job summary

OPCO Skilled Management in Farmington, NM seeks a licensed social worker to support residents in a skilled nursing setting. You will complete social histories, assess psychosocial needs, and help develop care plans in line with NASW codes and regulatory requirements.

Qualified candidates hold a Bachelor’s in Social Work or related field, are certified/licensed in the state, and have at least one year of supervised healthcare social work with geriatric populations; nursing experience is a plus.

Qualifications

• Certified, licensed, or registered in the state of practice is required.
• One year of supervised social work experience in a healthcare setting with geriatric individuals.
• Minimum of 2 years of nursing experience in a Skilled Nursing Facility preferred.

Responsibilities

• Completes social history and psychosocial assessment for each resident; uses MDS to guide care plan.
• Maintains accurate documentation complying with regulations and corporate policy.
• Supports residents’ right to self-determination and educates about alternatives.
• Develops an interdisciplinary care plan addressing psychosocial needs and goals.
