Job summary

AltaMed Health Services Corporation in Covina, CA is seeking a Social Worker (PACE) to provide psychosocial assessment, care planning, coordination of care, and discharge planning within the PACE program. You will engage in individual and group interventions, family contacts, and education as part of an interdisciplinary team.

Minimum qualifications include an MSW and at least 2 years of relevant experience; bilingual abilities are strongly preferred.

Qualifications

• Master's Degree in Social Work (MSW) from an accredited university.
• Minimum of 2 years of experience in case management, social advocacy, and/or mental health with geriatric populations.
• Bilingual Spanish/English, Chinese/English, or another second language preferred.
• Valid BLS certification or higher per AHA/Red Cross guidelines.

Responsibilities

• Provide psychosocial assessment, care planning, coordination of care, and discharge planning for PACE participants.
• Deliver individual and group interventions, family contacts, education, resource mobilization, and advocacy.
• Collaborate with an Interdisciplinary Team to optimize health outcomes and quality of life.
