Job summary

AltaMed Health Services Corp. is seeking a Social Worker, MSW, to provide psychosocial evaluation, care coordination, and discharge planning for the PACE program in Covina, CA. The MSW collaborates with the interdisciplinary team to optimize health and quality of life for participants.

The role requires an MSW, at least 2 years in geriatric case management or related fields, and preferred bilingual abilities. A valid BLS certification is required.

Qualifications

• MSW degree from an accredited university is required.
• Minimum 2 years in case management, social advocacy, and/or mental health with geriatric population.
• Bilingual Spanish/English or Chinese/English strongly preferred.
• Valid BLS certification per AHA/Red Cross guidelines.

Responsibilities

• Provide psychosocial evaluation, care planning, and coordination for PACE participants.
• Engage in individual and group interventions, family contacts, and education.
• Mobilize resources, identify behavioral health needs, and advocate for participants.
• Collaborate with Interdisciplinary Team to optimize health and quality of life.
