AltaMed in Covina, CA seeks a Social Worker for the PACE program to provide psychosocial assessments, care planning, and care coordination for participants and their families.

The role requires an MSW and at least 2 years of related experience; bilingual abilities (Spanish/English, Chinese/English, or other) are strongly preferred. Valid BLS certification or higher is required, with opportunities for professional growth within a community-focused clinic.

Qualifications

• MSW from an accredited university is required.
• Minimum of 2 years of experience in case management, social advocacy, and/or mental health, with the geriatric population.
• Bilingual Spanish/English, Chinese/English, or another second language strongly preferred.
• A valid BLS certification or higher per AHA/Red Cross guidelines.

Responsibilities

• Provide psychosocial assessment, care planning, coordination of care, and discharge planning for PACE participants.
• Utilize individual and group modalities, family and collateral contacts, and participant/family education; mobilize resources and advocate as needed.
• Collaborate within the Interdisciplinary Team to optimize health outcomes and quality of life for PACE participants, considering lifespan development and social determinants of health.
