Job summary

Outfield Healthcare Partners is seeking a licensed Social Worker to foster a supportive climate and provide psychosocial assessments for residents in a skilled nursing setting. You will develop and aid in access to services in line with NASW ethics and applicable laws.

The role requires a social work license, a bachelor’s degree in social work or related field, and experience with geriatric populations in healthcare.

Qualifications

• Certified, licensed, or registered in the state of practice.
• Bachelor’s Degree in Social Work or Human Services field.
• One year of supervised social work experience in a healthcare setting working with geriatric individuals.
• Minimum of 2 years of nursing experience in a Skilled Nursing Facility preferred.

Responsibilities

• Completes social history and psychosocial assessment for each resident identifying social, emotional, and psychological needs; uses MDS to guide the care plan.
• Maintains accurate and timely documentation per federal/state regulations and center practices, including care plans and social service notes.
• Supports resident self-determination; documents refusals and educates on alternatives as appropriate.
• Participates in an interdisciplinary care plan addressing psychosocial needs and goals.
