Job summary

Philosophy Care is seeking a devoted Social Worker to support geriatric residents in our skilled nursing facility. The role focuses on planning, organizing, developing, evaluating, and directing the social services program for the geriatric population.

Responsibilities include assessing needs, developing care plans, participating in interdisciplinary meetings, discharge planning, referrals to social services, and ensuring residents' rights are explained and respected.

Qualifications

• Bachelor’s degree in social work required.
• Experience in Long Term Care/Subacute is a plus.
• Knowledge of state and federal regulations.
• Ability to plan and direct social services processes.

Responsibilities

• Maintain and improve residents' ability to manage daily physical, mental, and psychosocial needs.
• Interview residents and families to identify social and emotional needs.
• Develop and revise care plans with goals for each resident.
• Participate in interdisciplinary care plan meetings.
• Participate in discharge planning.
• Refer residents to social service agencies when needed.
• Provide a positive image of the facility to residents and families.
• Explain resident rights and related information.
