You will conduct assessments, support families, collaborate with an interdisciplinary team, and connect residents with community resources, making a meaningful difference daily.

Qualifications

• Master's degree in social work preferred.
• Current licensure or certification as a Social Worker in the state of employment.
• Experience in Long-Term Care, geriatrics, or related field.
• Strong assessment, counseling, and crisis intervention skills.
• Excellent communication and interpersonal abilities.
• Ability to work collaboratively as part of a multidisciplinary team.
• Empathy, compassion, and commitment to person-centered care.

Responsibilities

• Conduct comprehensive psychosocial assessments identifying residents' social, emotional, and environmental needs, strengths and preferences.
• Collaborate with residents, families, and the interdisciplinary team to develop individualized care plans addressing social, emotional, and psychological concerns.
• Provide counseling and emotional support to residents and families, guiding coping strategies and transitions.
• Advocate for residents' rights and preferences in care planning and decision-making processes.
• Connect residents and families with community resources and advocacy organizations to address social, financial, and legal needs.
• Provide crisis intervention and support during challenging situations, such as grief or changes in health status.
