Job summary

InnovAge is seeking a Social Services Supervisor to lead the psychosocial services within the Interdisciplinary Team (IDT). The role ensures MSWs complete regulatory assessments, care plans, and documentation per PACE, state, and InnovAge standards, with SWAs meeting quality expectations.

The position supports safety and SD OH interventions, handles complex cases, and acts as a liaison among IDT, Center Triad, and community partners. Travel may be required.

Qualifications

• Master’s degree in social work required.
• Licensed Clinical Social Worker (LCSW) in CA required.
• Two years’ supervising social services staff.
• Three years’ social work experience in medical/healthcare settings.

Responsibilities

• Provide day-to-day supervision and performance management for MSWs and SWAs.
• Ensure regulatory compliance and timely documentation.
• Oversee complex cases and inter-disciplinary collaboration.
• Lead quality improvement and audit activities.
• Coordinate housing, safety, and SDOH interventions.
