Job summary

Golden Valley Health Centers in Modesto, CA is seeking a dedicated Social Worker to support Central Valley PACE participants, conducting screening, assessments, care planning and coordination.

You will provide individual and family counseling, refer to community resources, document care plans, and collaborate with the multidisciplinary team to ensure continuous services.

Schedule is Monday–Friday, 8:00am–5:00pm, with a pay rate of $35.25–$37.01 per hour and a full benefits package.

Qualifications

• Master's degree in Social Work and valid BLS CPR certification
• Excellent oral and written communication skills
• Bilingual English/Spanish preferred and culturally sensitive

Responsibilities

• Participates in multidisciplinary team assessments, care planning, and periodic reassessments
• Maintains current written case management records and reassessments
• Coordinates 24-hour care delivery and provides individual and family counseling
• Refers participants to community agencies and advocates for participants
• Documents participant changes in the medical record and supports discharge planning
• Engages with staff to promote teamwork and continuity of care
