LIFE Pittsburgh is seeking a qualified Social Worker to support an interdisciplinary team in delivering psychosocial assessment and care planning for older adults enrolled in the LIFE program. Master’s Degree in Social Work with geriatric focus.
Minimum of 1 year geriatric experience in hospital, long-term care, or community setting.
Experience coordinating community resources and services.
Experience relating to the geriatric population and their family members.

Responsibilities

Provide psychosocial evaluation, planning, and ongoing services for LIFE Pittsburgh Participants.
Educate participants and families, advocate for needs, and mobilize community resources.
Collaborate with an Interdisciplinary Team to optimize health and quality of life.
Support enrollment, and provide dis-enrollment education when required.
