Job summary

Outfield Healthcare Partners is seeking a Social Services Designee to support residents' individuality, independence, and dignity. You will identify psychosocial needs and coordinate with the care team to ensure compliance with regulations and NASW Code of Ethics.

Responsibilities include completing social histories, guiding MDS sections for care planning, documenting thoroughly, advocating for resident rights, and educating residents and families on care options within a interdisciplinary team.

Qualifications

• Certified, licensed, or registered in the state of practice (required)
• Bachelor’s Degree in Social Work or related human services field (required)
• 1 year of supervised social work experience in a healthcare or geriatric setting
• Skilled Nursing Facility experience preferred

Responsibilities

• Complete social histories and psychosocial assessments for each resident
• Accurately complete and utilize MDS sections to guide care planning
• Maintain thorough, timely documentation in compliance with regulations and company policy, including: Care plans, Progress notes, Behavior tracking, PASARR and discharge planning, Advance directives and psych service documentation
• Advocate for resident rights and support self-determination
• Educate residents and families on care options and alternatives
• Collaborate with the interdisciplinary care team to develop and implement individualized care plans
• Monitor and document cognitive, emotional, and psychosocial changes

Education

Bachelor's Degree in Social Work or related human services field
