Job summary

Community LIFE New Kensington, Pennsylvania, is seeking a Social Worker (MSW) to manage a caseload of participants aged 55+, perform psycho-social assessments, and provide interventions to help maintain independence in the community.

The Social Worker serves as primary contact for utilization case management of acute inpatient admissions and transfers, with a Monday–Friday schedule 7:30am–4:00pm, no nights or weekends.

Qualifications

• MSW from an accredited school of social work.
• Minimum of one year experience with frail or elderly population; geriatric setting preferred.
• Strong communication and interpersonal skills; empathy and adaptability required.

Responsibilities

• Maintain a caseload of Community LIFE participants aged 55+ and provide psycho-social assessments and interventions.
• Serve as primary contact and perform utilization case management for acute inpatient admissions and transfers.
• Coordinate with families and care teams to support independence in the community.

About this position

Position Summary: Our Social Workers maintain a caseload of Community LIFE participants (adults over age 55) and provide them and their families with psycho-social assessments and interventions related to the maintenance of the participants in the community. As the primary contact, the Social Worker performs utilization case management for all acute inpatient admissions and transfers. This position is Monday-Friday 7:30am-4:00pm with no nights or weekends!

Location: Lower Burrell, PA

Required Education: Master’s Degree in Social Work (MSW) from an accredited school of social work.

Required Experience: Must have a minimum of one year experience working with the frail or elderly population. Geriatric social work experience preferred in a hospital, long term care or community setting.

Required Skills:

• Good verbal and written communication skills.
• Ability to work with people from various socio-cultural backgrounds in an effective manner.
• Ability to accept and utilize supervision.
• Ability to work independently and be self-directed.
• Good problem-solving skills.
• Must have a compassionate and empathetic attitude for the frail older adults.
