Job summary

Community LIFE New Kensington, Pennsylvania, is seeking a Social Worker (MSW) to manage a caseload of participants aged 55+, perform psycho-social assessments, and provide interventions to help maintain independence in the community.

The Social Worker serves as primary contact for utilization case management of acute inpatient admissions and transfers, with a Monday–Friday schedule 7:30am–4:00pm, no nights or weekends.

Qualifications

• MSW from an accredited school of social work.
• Minimum of one year experience with frail or elderly population; geriatric setting preferred.
• Strong communication and interpersonal skills; empathy and adaptability required.

Responsibilities

• Maintain a caseload of Community LIFE participants aged 55+ and provide psycho-social assessments and interventions.
• Serve as primary contact and perform utilization case management for acute inpatient admissions and transfers.
• Coordinate with families and care teams to support independence in the community.
