Job summary

Community LIFE is seeking a Social Worker (MSW) to manage a caseload of older adults, conduct psycho-social assessments, and coordinate interventions to support independence at home.

The role requires an MSW and at least one year of elder-care experience, with a schedule Monday–Friday 7:30am–4:00pm and no evenings or weekends.

Qualifications

• Master’s Degree in Social Work (MSW) from an accredited school.
• Minimum of one year experience with frail or elderly population.
• Geriatric social work experience preferred in hospital, long-term care or community setting.

Responsibilities

• Maintain a caseload of Community LIFE participants and provide psycho-social assessments and interventions.
• Perform utilization case management for acute inpatient admissions and transfers.
• Monday–Friday 7:30am–4:00pm with no nights or weekends; communicate with families and participants.

Salary Range: $58,000.00 To $65,000.00 Annually

Position Summary: Our Social Workers maintain a caseload of Community LIFE participants (adults over age 55) and provide them and their families with psycho-social assessments and interventions related to the maintenance of the participants in the community. As the primary contact, the Social Worker performs utilization case management for all acute inpatient admissions and transfers. This position is Monday-Friday 7:30am-4:00pm with no nights or weekends!

Location: Lower Burrell, PA

Required Education:Master’s Degree in Social Work (MSW) from an accredited school of social work.

Required Experience:Must have a minimum of one year experience working with the frail or elderly population. Geriatric social work experience preferred in a hospital, long term care or community setting.

Required Skills:

• Ability to work with people from various socio-cultural backgrounds in an effective manner.
• Ability to accept and utilize supervision.
