Responsibilities

• Evaluate and assess residents' social service needs and concerns.
• Develop relationships with residents and families; address needs promptly.
• Initiate social service charting and activity programs at admission.
• Monitor residents quarterly and complete required charting.
• Ensure rights are maintained through observation and assessment.
• Complete quarterly progress notes, social service log, and attend care plan meetings.
