Job summary

Bethesda Health Group in Missouri is seeking a Social Services Coordinator to assess residents' social service needs, build strong relationships with residents and families, and ensure timely resolution of concerns. You will initiate charting and develop activity plans to support residents' physical and mental well‑being from admission onward.

The role requires documenting quarterly progress notes, maintaining social service logs, and actively participating in Resident Care Plan meetings.

Qualifications

• Bachelor's degree in social work is required.
• Must have knowledge of growth and development and show expertise for geriatric population.
• Two years experience preferred.
• Geriatric and Social Work experience preferred.
• Excellent organizational and communication skills.
• Ability to perform necessary documentation in writing and electronically.

Responsibilities

• Evaluate and assess residents' social service needs and concerns.
• Develop relationships with residents and families.
• Initiate social service charting and activity plans.
• Monitor residents quarterly and complete charting as required.
• Complete quarterly progress notes and social service logs.
• Participate in Resident Care Plan meetings.
