Job summary

Bethesda Health Group in Missouri seeks a qualified Social Worker to provide goal-oriented, person-centered services for residents in a long-term care setting. You will assess needs, coordinate social services, and support residents and families through planning and documentation.

As part of an interdisciplinary team, you’ll assist with admissions, Medicaid applications, and transportation scheduling, ensuring resident rights and timely notes while maintaining compassionate, professional care.

Qualifications

• MSW required.
• High school diploma or equivalent required.
• Geriatric care knowledge and experience preferred.
• Knowledge of resident rights and Medicaid processes preferred.
• Strong written and electronic documentation skills.

Responsibilities

• Provides goal-oriented and person-centered social services for residents.
• Maintains charting and completes required documentation.
• Coordinates with interdisciplinary team to address resident and family needs.
• Assists with admissions, Medicaid applications, and transportation scheduling.
• Participates in Resident Care Plan meetings and follow-up notes.
• Ensures resident rights and advance directives are reviewed and updated.
• May perform additional duties as assigned.
