Job summary

Vered Healthcare Group seeks a Speech-Language Pathologist (SLP) for a Skilled Nursing Facility. Requires a master’s degree in Speech-Language Pathology, eligibility for Praxis, and state licensure.

Responsibilities include evaluating residents, creating therapy plans, collaborating with the care team, documenting progress, and educating patients and families about therapy goals, swallowing safety, and communication in a geriatric setting.

Qualifications

• Graduate degree in Speech-Language Pathology from an accredited program.
• CCC credential or eligible for licensure.
• Eligible to apply for state license to practice.
• Pass Level 2 fingerprinting background check required.
• Experience with adult and geriatric care.
• Excellent communication and collaboration skills.

Responsibilities

• Evaluate residents for speech, language, cognitive, and swallowing disorders.
• Develop and implement individualized therapy plans to meet rehabilitation goals.
• Collaborate with nurses, therapists, dietitians, and social workers to plan care.
• Maintain thorough documentation of assessments, treatment plans, and progress.
• Educate residents and families about therapy goals and swallowing management.
• Provide supports in palliative care focusing on communication needs.
