Prima CARE PC is seeking a Speech-Language Pathologist to work in a Skilled Nursing Facility. The role involves evaluating residents, designing therapy plans, and collaborating with an interdisciplinary team to improve communication and swallowing outcomes.

The position requires a graduate degree in Speech-Language Pathology and CCC certification, with strong experience in adult and geriatric care. This on-site role supports patient and family education and end-of-life communication needs.

Formación

• Graduate degree in Speech-Language Pathology from an accredited program.
• Certificate of Clinical Competence (CCC).
• Strong clinical knowledge and experience in adult and geriatric care.

Responsabilidades

• Evaluate residents for speech, language, cognitive, and swallowing disorders.
• Develop and implement individualized therapy plans to meet residents' rehabilitation goals.
• Collaborate with nurses, occupational therapists, physical therapists, dietitians, and social workers to create a care plan.
• Maintain thorough records of assessments, treatment plans, progress notes, and discharge summaries.
• Educate residents and families about therapy goals, strategies to maximize quality of life, and swallowing safety.
• Provide support and education for palliative care focusing on communication and swallowing needs.
