Job summary

Manchester Center for Rehabilitation and Healing is seeking a licensed Speech Language Pathologist (SLP) for full-time and PRN positions in a Skilled Nursing facility. The ideal candidate will evaluate and treat independently, provide functional therapy, and support geriatric residents’ communication and swallowing needs.

Responsibilities include assessing patients, creating goals, documenting progress, and collaborating with the rehabilitation team to ensure patient-centered care.

Qualifications

• Graduated from an accredited Speech-Language Pathology program.
• Valid professional license and TN registration.
• Experience in long-term care and/or geriatric care preferred.

Responsibilities

• Assess referred patients requiring rehabilitation and provide direct patient care.
• Develop and administer treatment plans with goals aligned to patient capacity.
• Document evaluations, progress notes, discharge summaries, and educate patients/families.
