Job summary

Western Reserve Area Agency on Aging is seeking an LPN Community Resource Consultant to assess individual needs and determine eligibility for programs. This position is essential in providing options and referrals to clients while ensuring their well-being.

Qualifications

• Current licensure or as a Licensed Practical Nurse with the Ohio Board of Nursing.
• One year of experience in home care, geriatrics, home health care, behavioral health, long-term care, or substitute approved by Ohio Department of Aging.
• Knowledge of major chronic illnesses, disease processes, normal aging, medical terminology.

Responsibilities

• Complete assessments to identify unmet needs and determine level of care.
• Educate consumers and stakeholders regarding eligibility of programs.
• Document in appropriate software systems according to requirements.

Skills

Excellent verbal and written interviewing and assessment skills

Knowledge of local resources for older adults

Excellent interpersonal and communication skills

Intermediate computer skills

Education

Current licensure as a Licensed Practical Nurse

One year of experience in home care or geriatrics
