Job summary

Waukesha County’s ADRC seeks an experienced ADRC Specialist to deliver information and assistance for older adults and people with disabilities. The role involves engaging with consumers to assess needs, provide options, and guide enrollment into Wisconsin’s Adult Long-Term Care programs.

Responsibilities include call-center support, home or off-site assessments, record-keeping, and collaboration with local health and social services. Remote work up to 2 days/week after prerequisites are met.

Qualifications

  • Bachelor's degree in sociology, social work, psychology, nursing, criminal justice, or a closely related field.
  • Two years of responsible work experience in social work, counseling, nursing, criminal justice, or a closely related field.
  • Master’s degree in social work, nursing, psychology, criminal justice, or a closely related field may substitute for one year of the work experience.
  • Social work certification may substitute for one year of work experience.
  • Valid driver’s license.

Responsibilities

  • Provide information and assistance to adults with disabilities, older adults and their caregivers via a call center.
  • Engage with consumers to evaluate needs and goals, including home assessments as needed.
  • Review options and next steps based on gathered information.
  • Assist consumers with enrollment steps into Wisconsin’s Adult Long-Term Care Programs.
  • Maintain meticulous records to support informed decision-making and reflect on practice.
  • Collaborate with a network of HHS and community service stakeholders.
