Job summary

FOX Rehabilitation in Piermont, NY is seeking motivated, licensed Physical Therapist Assistants to deliver one-on-one care in patients' homes and senior living communities through our Geriatric House Calls model.

You’ll work with a supervising PT, guide patients through exercises, document visits in our EMR, and coordinate with care teams and family POAs. Eligible candidates have PTA license, CPR certification, and proficient computer skills.

Qualifications

• Valid Physical Therapist Assistant license in state(s) of practice.
• Graduate of an accredited PTA program.
• Current CPR certification.
• Basic computer skills; proficient in Microsoft Office.

Responsibilities

• Deliver individualized treatment plans established by supervising PT, adjusting activities based on patient response.
• Instruct, motivate, and safeguard patients through therapeutic exercises and functional activities.
• Document visits accurately and on time within FOX’s EMR system.
• Coordinate with patients, POAs, and community referral sources; assist coverage when needed.
