Job summary

FOX Rehabilitation is seeking motivated, licensed Physical Therapist Assistants to deliver one-on-one, in-home and senior living care under supervision of a PT. The Geriatric House Calls™ model helps older adults regain strength, mobility, and confidence in their homes and communities.

Qualifications

• Valid Physical Therapist Assistant license in the state(s) of practice, or eligibility to obtain.
• Graduate of an accredited Physical Therapist Assistant program.
• Current CPR certification.
• Basic computer skills; proficient in Microsoft Office.

Responsibilities

• Deliver individualized treatment plans established by the supervising PT, adjusting activities based on patient response.
• Instruct, motivate, and safeguard patients through therapeutic exercises and functional activities.
• Document visits accurately in FOX’s EMR system according to Clinical Documentation standards.
• Coordinate with patients, POAs, and referral sources and assist with coverage as needed.
