Job summary

FOX Rehabilitation is seeking motivated Physical Therapist Assistants to deliver hands-on, one-on-one care in homes and senior living communities in New Jersey. You’ll work closely with a supervising Physical Therapist to implement individualized treatment plans and guide patients through exercises, promoting strength, mobility, and confidence.

A collaborative, interdisciplinary team supports mentorship and growth within the Geriatric House Calls model.

Qualifications

• Valid Physical Therapist Assistant license in the state(s) of practice, or eligibility to obtain.
• Graduate of an accredited Physical Therapist Assistant program.
• Current CPR certification.
• Basic computer skills; proficient in Microsoft Office.

Responsibilities

• Deliver individualized treatment plans established by your supervising Physical Therapist.
• Instruct, motivate, and safeguard patients through therapeutic exercises and functional activities.
• Document visits accurately and on time within FOX’s EMR system.
• Stay connected with patients, POAs, and community referral sources, and coordinate coverage when needed.
