Job summary

On Lok in San Jose, CA is seeking a licensed Physical Therapist to provide and supervise treatment for program participants under clinical supervision.

You will evaluate the participant's physical condition, develop and implement treatment plans, supervise the physical therapy assistant and rehab aide, and collaborate with an interdisciplinary team to optimize outcomes. The position offers an hourly salary range of $53.55–$71.40.

Qualifications

• Graduate of an approved university course in Physical Therapy.
• Licensed by the Physical Therapy Examining Committee of the California Board of Medical Quality Assurance.
• Minimum one to two years of experience working with the elderly or frail population.
• Flexibility, initiative and ability to work in a multidisciplinary team.
• Computer literate.

Responsibilities

• Evaluates the participant's physical condition and functional status; establishes a realistic, measurable goal and sets up a treatment plan to administer physical therapy treatment.
• Completes initial and re-evaluation reports in a timely manner; documents participant's progress and other relevant information.
• Supervises the physical therapy assistant and rehab aide; provides training/instruction to other staff members, participants, and caregivers, in the Day Health Center and/or at home, as needed.
• Participates actively in the interdisciplinary team by offering clinical expertise in team planning, problem solving, and processes.
• Other related duties as assigned.
