Job summary

Lorian Health in San Jose, California is seeking a Home Health Physical Therapy Assistant to deliver skilled therapies under the supervision of a licensed Physical Therapist. You’ll help patients improve mobility, balance, gait, and independence in their own homes.

This pay-per-visit role offers $50 to $70 per visit, depends on visit type and experience. You must graduate PTA school, be licensed in California, hold CPR certification, and have a valid driver's license.

Qualifications

• Graduate of a physical therapy assistant program approved by a recognized accrediting body.
• Licensed/registered to practice as a PTA in California.
• Valid driver's license and insured personal vehicle.
• Minimum of one year of home health or hospice experience.
• Current CPR certification.

Responsibilities

• Provide skilled physical therapy interventions as ordered by the physician and under the supervision of a licensed Physical Therapist.
• Help patients improve mobility, balance, gait, and independence at home.
• Educate patients and families on home exercise programs and safety strategies.
• Monitor patient condition and communicate changes to the supervising PT and care team.
• Assess home environment and recommend modifications to promote safety.
• Maintain accurate clinical documentation per policies and regulatory requirements.
• Collaborate with interdisciplinary team and participate in education/in-service programs.
