Job summary

Lorian Health is seeking a Home Health Physical Therapy Assistant (PTA) for per diem visits in the San Jose area. You will provide skilled PT interventions under a licensed Physical Therapist, helping patients improve mobility, balance, gait, and independence in their own homes.

Key responsibilities include patient education, home exercise programs, environment assessment for safety, and thorough clinical documentation.

Qualifications

• Graduate of a physical therapy assistant school approved by the AMA Council of Medical Education or its equivalent.
• Licensed/registered to practice as a PTA in the state of practice.
• Valid driver’s license and insured personal vehicle per Lorian requirements.
• Minimum of one year of recent Home Health or Hospice setting experience.
• Possesses and maintains current CPR certification.

Responsibilities

• Provide skilled physical therapy interventions as ordered by the physician and under the direction and supervision of a licensed Physical Therapist.
• Support patients in improving mobility, balance, gait, strength, and functional independence while helping minimize physical limitations.
• Educate and instruct patients and families/caregivers on home exercise programs, balance and gait training, and strategies to support continued progress.
• Monitor patient condition and progress, communicating changes and concerns with the supervising Physical Therapist, physician, case manager, and care team.
• Assess the home environment and recommend appropriate modifications or strategies to promote patient safety and independence.
• Maintain accurate, complete, and timely clinical documentation in accordance with Lorian policies, procedures, and regulatory requirements.
• Collaborate with the interdisciplinary care team, participate in education and in-service programs, and provide guidance to other healthcare personnel when appropriate.
